Learning Disabilities

The practice offers an annual assessment for all patients with Learning Disabilities which is supported by the practice Doctors and Nurses. At this appointment you can ask any questions about your health and your GP can refer you to a specialist if it is needed. You will be able to have an influenza vaccination to help keep you safe this winter and other checks that my be due such as a smear test.  A general health check will be done to make sure you are getting the best treatment and our nurses can give you easy read leaflets to help you understand the tests that you may need. Your carer can also get support from our team too. You will leave with a Health Action Plan that you can follow to help with your health.

Please ensure you attend the surgery for your health check when you are invited.

We have recently had a Learning Disability Health Assessment Audit carried out at our surgery to evaluate how well we do with our Health Assessments. You will be pleased to know, we are at the highest level which is GOLD for the second year running. This means we are supporting patients with learning disabilities to the best of our ability.
